How to schedule an appointment

If your doctor or healthcare provider has referred you to us, we need to obtain a written prescription from the provider before scheduling your appointment.  If you are using health care insurance, we will schedule your appointment after obtaining insurance benefits and authorizations.

If you do not have a prescription from a referring doctor, we will schedule a new patient consultation appointment in our office. When you call to schedule, please have your insurance card ready.  

Preparing for your appointment

Step 1. Prepare a list of medications you are taking if applicable.

Step 3. Please read all the information in the Patient Information brochure or Appointment form carefully, complete all forms provided by our staff, and bring all forms with you on the day of your appointment.

 

Stop taking the following medications prior to the procedure:

o        Aspirin for seven (7) days

o        Anti-inflammatory medications for three (3) days (e.g., Ibuprofen, Naproxen, Aleve, Celebrex)

o        Narcotic pain medication and/or sedatives for six (6) hours (e.g., Vicodin, Norco, Valium)

o        If you are on diabetes medications or blood thinners (e.g. Coumadin, Heparin, Plavix), follow the special instructions provided by our staff in the Appointment form.

 

Step 4. On the day of the procedure:

·         Do not eat for eight (8) hours prior to your procedure.  Do not take liquids four (4) hours prior to your procedure.  Sips of water with medication are acceptable any time.

·         Arrive one hour prior to your appointment time.

·         Bring all completed forms and provide to Dr. O’Neill.  Electronic copies are available:

o        New patient history (4 pages)

o        New patient consultations (2 pages)

o        Established patient history (1 page)

o        Financial and Privacy consent form (3 pages)

·         Bring MRI, CT and X-Ray films to all appointments.

·         Have a driver if you are having an outpatient procedure. Arrange for a driver to pick you up approximately one (1) hour after your procedure time (no taxis or public transportation).

·         Bring your driver’s license or official identification.  If applicable, bring your insurance card and cash, check or credit card for your co-payment.

·         Leave valuables and jewelry at home.  Wear loose comfortable clothing.  If applicable, bring glasses/contact lenses and a case to contain them.

·         Call us if you develop symptoms of a cold, flu, other infection, if you are placed on antibiotics, or if you think you might be pregnant.

·         Provide a two (2) business day advanced cancellation notification at 415-600-7830.  Cancelled appointments within two (2) business days will incur a cancellation fee.
